Initial Comments:
Based on the findings of an onsite unannounced revisit completed December 14, 2022, Dialysis Clinic, Inc. - Beaver had corrected the deficiencies cited under 42 CFR, Part 494.62, Subpart B, Conditions for Coverage of Suppliers of End-Stage Renal Disease (ESRD) Services - Emergency Preparedness. The deficiencies were cited as a result of a Medicare recertification survey completed November 7, 2022.
